A company is looking for a Telephonic RN Nurse Care Manager.
Key Responsibilities
Manage a caseload of high-risk patients, providing clinical
coordination and care management
Collaborate with patients and multidisciplinary teams to develop
and implement individualized care plans
Conduct extensive telephone assessments and prioritize nursing
interventions based on patient conditions
Required Qualifications
Active R.N. License
2 or more years of direct home health experience or care
management
Knowledge of Medicare regulations and home care standards
Ability to perform extensive telephone assessments
Self-motivated and organized with the ability to meet deadlines
