A company is looking for an RN Specialist in Complex Case
Management.
Key Responsibilities
Engages with members and providers to manage complex medical
conditions through telephonic outreach
Conducts clinical assessments and collaborates with an
interdisciplinary care team to develop comprehensive care plans
Monitors and updates care plans while ensuring compliance with
documentation standards and promoting cost-effective outcomes
Required Qualifications, Training, and Education
Current registered nurse license in Minnesota with no
restrictions
Five years of relevant clinical care experience
CCM Certificate or ability to obtain within three years of starting
the position
Experience in telephonic engagement and keen business skills
Proficiency in computer applications
