Job Description:
A company is looking for a Care Navigator.
Key Responsibilities
Build trusting relationships with individuals living with dementia
and their caregivers, acting as their care team champion
Provide compassionate support, care coordination, and education for
Dyads with the interdisciplinary team
Manage care coordination, administer assessments, and support Dyads
through various stages of care
Required Qualifications
Two or more years of experience as a Care Navigator, Community
Health Worker, Case Manager, Medical Assistant, or personal
experience caring for someone with dementia
Experience working with older adults, particularly those with
cognitive disorders
Ability to work in multidisciplinary care teams
Experience navigating EHRs and encounter notes
Based in Cleveland, Ohio
