Job Description:
A company is looking for a Health Navigator to support patients
in a chronic care remote monitoring program.
Key Responsibilities
Monitor and screen incoming alerts, calls, and chat
communications
Conduct patient onboarding and schedule audio-visual calls with
patients
Report patient concerns to the Registered Nurse and assist with
technology troubleshooting
Required Qualifications
High school diploma or GED required
Experience in a clinical setting, preferably in a patient-facing
role
Familiarity with electronic health records and patient
documentation
Strong computer skills and ability to learn new technology
Ability to work in a HIPAA compliant environment and handle
stressful situations
